Health and social care in East Sussex, working together with you

Partners across the NHS in East Sussex, East Sussex County Council, Healthwatch East Sussex and the voluntary and community sector have been working together to draw up a Health and Social Care Plan for our county. The draft plan sets out the health and social care needs of our East Sussex residents, and how we intend to drive the transformation needed to meet them over the next 3-5 years.

The plans are a continuation of the progress we’ve already made over the past few years joining up health and social care in East Sussex. For example the award-winning i-Rock services for young people, Health and Social Care Connect (HSCC) which is a fully integrated central point for health and adult social care enquiries, our community crisis response and falls prevention services.

Our plans and priorities have been informed by what local people have told us is important to them. Through our new joint plan we want to continue our work to improve our population’s health and wellbeing and reduce health inequalities, by breaking down barriers between health and social care to deliver the right care and support, at the right time, in the right places for local people.

The draft plan focuses on our agreed priorities and next steps for transformation and integration between health and social care; prevention, children and young people, community services, urgent care, planned care and mental health.

There is growing demand on both NHS and social care services. More and more local people will require support and care for long term conditions. By joining up the care we provide we will be better able to support people to live as independently as possible and achieve the best possible outcomes for them.

Our East Sussex plan also forms part of a wider health and social care strategy for the whole of Sussex. The Sussex-wide strategy is a response to the national NHS Long Term Plan and covers joint priorities for East Sussex, West Sussex and Brighton and Hove, as well as Sussex-wide clinical priorities. For example, maternity services, cancer, stroke, falls and fractures.

Over the next few weeks the draft East Sussex Health and Social Care plan will go through each of our partner’s formal sign off processes.
