New online tool to help people get occupational therapy support

People in East Sussex who are disabled or have a health condition that affects their mobility can now use a new online self-assessment for occupational therapy support.

The new tool has been launched by the East Sussex County Council Being Digital Programme. It means that adults in East Sussex can start the occupational therapy assessment journey themselves, telling us about their needs, how it impacts their wellbeing, and what they want to achieve.

If you’re disabled or have a health condition that affects your mobility, you may find it difficult to access or use your home safely, get out and about, or have difficulty with other everyday activities. If you are assessed and eligible, this assessment will look at whether changes can be made to your home to make it easier for you to get around. We can also provide equipment to help you with important daily tasks you’re struggling with.

Once the form is submitted, it’s reviewed within the Health and Social Care Connect team (HSCC) at the Council. If you’re eligible, then the occupational therapy team will follow up by phone or email to continue the process.

The new form makes it convenient for family members to complete an assessment together, at a time that works for you, and without being under pressure to give lots of answers in one go over the phone.

Health and care professionals can also fill in the form.

The self-assessment form is available 24/7 via the Adult’s Portal on the East Sussex County Council website.
