About us

We are a partnership of organisations who plan and deliver health and care in East Sussex. By working together, we aim to deliver sustainable health and social care that better meets the needs of local people, offering high quality care at the right time, in the right place.

We’ve been working together for several years to help improve the health and wellbeing of local people, reduce health inequalities across East Sussex, and break down barriers between health and social care.

Our partnership is where public health, primary care, community health, mental health, social care, housing and hospital-based services work together on shared priorities. The organisations involved are:

  • East Sussex County Council
  • Our new NHS Sussex Integrated Care Board
  • East Sussex Healthcare NHS Trust
  • Sussex Community NHS Foundation Trust
  • Sussex Partnership NHS Foundation Trust
  • Primary Care Networks
  • District and borough councils
  • Healthwatch
  • Voluntary, community and social enterprise (VCSE) organisations

We also work with a range of public and private organisations that impact on the health, care and wellbeing of our population including East Sussex Fire and Rescue Service, South East Coast Ambulance Service, education providers, and registered landlords.

Together, we’ve identified a set of priorities based on your needs. We have a plan called ‘Healthy lives, healthy people’ that sets out our joint priorities for the health and wellbeing of the county’s residents – especially for those people, places and communities who have the worst health outcomes – now and in the future. Healthy lives, healthy people

How we’ll achieve these goals is described in what’s called our ‘shared delivery plan’, also known as ‘Our Plan for Our Population’: Our plan for our population. This ties together plans for improvements across Sussex with our local plans to deliver Healthy Lives, Healthy People, and with similar plans for West Sussex and Brighton Hove.

Working together across Sussex

As well as working together in East Sussex, we’re also part of the Sussex Health and Care Partnership Integrated Care System (ICS). This is made up of health and care organisations working together across East Sussex, West Sussex and Brighton and Hove. Working as part of the ICS means we can plan and co-ordinate services at an even wider scale where this makes sense to help meet the needs of our population.

This website and our newsletter aims to inform you on our work to:

  • bring change to how you access services and support
  • increase prevention and early intervention
  • better join up health and care services for children and young people, mental health, urgent care, planned care, and community services for adults and older people with long term complex care needs and multiple conditions.

We’ll also share a range of news and topical features on local health and social care every month.
